people impacted by death, dying and loss often suffer a social death causing social isolation and loneliness.<\/p>\n<p>By ensuring that people living with an advanced illness and frailty remain connected to people in the places where they normally live, work, play and pray, they are included and can contribute to their communities.<\/p>\n<p>The same social death applies to informal carers
